Mine is pretty simple as well.
First section is a reflective journal.
Second section is a scanned copy of my dailies (I write my daily calendar and top priority tasks on my planner which I keep in front of me during the day. Weekly I scan them and transclude them into my daily notes for the previous week.)
Third section is a list of links to any date-referenced notes for that day (minutes of meetings, event notes, etc.
My daily note therefore stands as a record of what happened that day - my thoughts/feelings, what I did, and minutes or date-centric notes.
